COBRA Case Management

COBRA case management is a set of services for people living with HIV/AIDS and those who are high risk negatives who are eligible for Medicaid insurance.  

We help to connect you to resources and coordinate your care. We advocate on your behalf to ensure that you have access to comprehensive medical care, health insurance, entitlements, housing, food, substance use services, dental and mental health providers, job training services, support groups, and treatment education. We also work with your collaterals. Our goal is to provide you with the tools to become stable and self sufficient.

Eligibility: You must have active Medicaid. Please note that some clients with active Medicaid may not be able to enroll immediately in COBRA. These include clients who are receiving services from:

• Another COBRA program
• Day treatment
• Scatter-site housing 1               
• Nursing Home
• Lombardi homecare services
