Become a Client

Client Registration/Intake effective June 1, 2009

The Client Registration/Intake hours are as follows:

Mondays: 11 am – 4 pm
Tuesdays: 10 am – 1:30 pm
Wednesdays: 4 pm – 6:30 pm
Thursdays: 10 am – 12:30 pm

Appropriate medical documentation is required for people living with HIV/AIDS requesting to become a client. The following types of medical documentation are accepted:

•    An M11Q form
•    A letter from a doctor including doctor’s medical license number
•    Current blood work results with physician’s name — including CD4 count and viral load results
•    Letter confirming results on HIV test from a testing site or medical provider
•    GMHC’s Medical Documentation Form (available at the 5th floor Reception Desk)
